• This bureau service is completely free of charge.
• Candidates must fill out the form in capital letters only.
• All enquiries and decisions must be made by both parties.
• The bureau, church, pastor, elders, or coordinator are not responsible for any disputes before or after the marriage.
• For any complaints or suggestions, please contact the bureau directly.
• The bureau is authorized to display or share this form with other bureaus or during marriage seminars.
• Forms will be issued only to baptized, born-again (Trinity) candidates. (Matthew 28:19–20)
• Incomplete forms will not be accepted under any circumstances.
• Signatures of the candidate, parents, and pastor (optional)
• No interaction or processing will be done without submission of the completed form.
• All information provided by the candidate must be true and verified by parents or relatives. The bureau, church, pastor, elders, or coordinator will not be responsible for any incorrect information.
• If the marriage is finalized, please inform the bureau.
• The bureau reserves the right to display the form whenever and wherever necessary.
